Tuesday 8:00 AM–4:00 PM, Wednesday 10:00 AM–6:00 PM, Thursday 9:00 AM–5:00 PM, Friday 8:00 AM–4:00 PM, Saturday 10:00 AM–6:00 PM (40 hours per week).
About the Role
Provides a comprehensive array of support focused on skill building and case management activities to help clients achieve a meaningful level of independence and well-being. Completes screenings and assessments with clients to develop person-centered care plans. Works in a team-based approach delivering rehabilitative supports, skill building for activities of daily living, resource access and engagement, psycho-education groups, socialization, and community integration. Maintains a caseload of at least 20 individuals in community or program settings.
Responsibilities
- Establish rapport and positive working relationships with clients while maintaining professional ethics and boundaries.
- Assess and teach daily living skills through modeling, prompting, and hands-on assistance.
- Develop, implement, and monitor person-centered plans throughout intake and discharge processes.
- Coordinate and assist clients with case management needs, including referrals, entitlements, money management, treatment, medical care, transportation, housing, wellness/nutrition, legal services, education, and employment.
- Identify and address potential barriers to client goals by linking them to appropriate services.
- Meet regularly with clients to review progress, mental health stability, and upcoming appointments.
- Support clients in learning local transportation and provide transportation as needed, including use of personal vehicle in community-based and shared-living programs.
- Conduct regular apartment/room inspections and assist clients in maintaining clean living spaces, including hands-on assistance.
- Develop and maintain relationships with families, natural supports, and external providers; advocate for clients as necessary.
- Facilitate or co-facilitate psycho-educational groups.
- Complete service entry documentation per contract and agency requirements.
- Administer breathalyzer tests and urinalysis samples as applicable.
- Conduct program rounds as indicated.
- Attend and participate in staff meetings, All-Treater’s meetings, rounds, and supervision.
- Maintain a clean, well-organized work environment and ensure program egresses and walkways are clear for safety.
- Report maintenance and safety issues; generate work orders as needed.
- Participate in fire and emergency evacuation drills and review emergency preparedness plans.
- Engage in Food Bank activities and other designated environmental tasks as assigned.
- Adhere to policies outlined in the staff handbook and program operations manual.
- Uphold HIPAA regulations and secure necessary releases for outside communication.
- Model culturally competent behavior with clients, providers, family, and staff.
- Maintain accurate electronic records and case files per programming, contractual, licensing, and accreditation requirements.
- Adhere to standards of the Council on the Accreditation of Rehabilitation Facilities.
- Participate in agency committee work and complete required trainings, in-services, and professional development opportunities.
Requirements
- Bachelor’s degree required with a minimum of one (1) year of experience; high school diploma considered with three (3) years of experience, including one (1) year working with Continuum.
- Prior experience working with people with persistent chronic mental illness and/or addiction.
- Valid driver’s license and reliable vehicle required for community-based and shared-living programs.
- Must maintain minimum required CT State automobile liability insurance on personal vehicle.
- CPR/First Aid certification (offered as part of orientation; employee responsible for maintaining ongoing certification).
- Proficient reading, writing, and oral skills, including computer proficiency and familiarity with electronic record keeping.
- Knowledge of the community support system and resources preferred.
