Case Manager (ICM)

About the Role

Under general supervision, the candidate will perform social services functions at an advanced level to individuals and their families. Assist in engagement, needs identification, care coordination, referrals and linkage, and follow-up with individuals on assigned caseload. Travel between Columbus, Ga and Americus, Ga is required. This position works with individuals with severe and persistent mental health diagnoses, developmental disabilities, and/or addictive diseases.

Responsibilities

• Perform advanced social services functions for individuals and families
• Engage with clients and identify needs
• Coordinate care and provide referrals/linkage to resources
• Follow up with individuals on assigned caseload
• Document interactions and communications
• Communicate with individuals, families, and community resources
• Transport clients as needed (requires valid driver’s license, insured vehicle, and clean driving record)

Requirements

• High School Diploma with 3-5 years' experience in behavioral health or a bachelor's degree in social work, psychology, mental health, sociology, counseling, or a related discipline (degree must be in one of these fields to be MHC eligible)
• Valid driver’s license, personal insured vehicle, and clean driving record

Preferred Qualifications

• Bachelor’s degree in a relevant field
• Experience working with individuals with mental disorders and/or addictive diseases

Schedule

• Full-time (40 hours per week)
• Monday through Friday, 8:00 AM to 5:00 PM

Pay

$33,072 per year
