Case Manager - Housing Program

Responsibilities

• Manage and monitor a rapid re-housing program for young adults
• Provide direct client services and maintain related documentation
• Implement the TIP Model with housing clients
• Provide supportive services to promote participants' self-sufficiency, integration into the community, and permanency in housing
• Perform administrative tasks involved in the review and maintenance of a caseload of program participants
• Work in both a general office environment and in the field/community where program participants reside

Qualifications

• Bachelor's degree in a related human service field required
• 2 years' experience in human service field, working with transition age youth preferred
• Valid NC Driver's License
