About the role

This is a full-time, hybrid position requiring travel to community-based meetings multiple times per week and remote work from a home office. The role involves in-person meetings, so applicants must live within the State of Indiana or within 10 miles of the state line.

Responsibilities

  • Provide information and education to help Individuals understand the Medicaid waiver process
  • Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals served
  • Help Individuals access services and supports they need to meet their goals
  • Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed

Who you’ll work with

Case managers work with Individuals on four Medicaid Waivers: Family Supports Waiver (FSW), Community Integrated Habilitation Waiver (CIH), Health & Wellness Waiver (H&W), and Traumatic Brain Injury Waiver (TBI).

Requirements

  • A Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field, or
  • A Bachelor's degree in an unrelated field and a minimum of one year of direct experience working with people with intellectual and/or developmental disabilities, or two years of experience working with the aging population
  • A valid driver's license, car insurance, and reliable transportation
  • The ability to pass thorough background screens
  • A designated home office setting with a computer, internet service, and smartphone
