About the role
Suburban Home Health is a joint venture between several Suburban Health Organization hospital systems in Indiana. This allows us to work as an extension of our partner hospital systems to service homebound patients in our local communities. As a Registered Nurse in home health, you are given the freedom and flexibility to manage your own schedule and provide care one-on-one to adult/geriatric patients in the home setting.
Responsibilities
- Manage and evaluate patients' care plans
- Provide wound care
- Perform infusion therapy
- Administer and manage medication
- Deliver education to patients and their families
- Typically perform 2 admission visits or 6 routine visits in a standard work day
