About the role

This position is available through a joint venture between several Suburban Health Organization hospital systems in Indiana. It allows for the extension of services to homebound patients within local communities.

Responsibilities

• Manage and evaluate patient’s care plans
• Provide wound care
• Administer and manage medication
• Deliver education to patients and their families

Qualifications

Must be a registered nurse with a current license to practice in the state of Indiana. Experience in home health care is preferred but not required. Excellent communication and interpersonal skills are essential.
