About the role
This is a full-time, hybrid position requiring travel to community-based meetings multiple times per week and remote work from a home office. The role requires in-person meetings, so applicants must live within the State of Indiana or within 10 miles of the state line.

Responsibilities
- Provide information and education to help Individuals understand the Medicaid waiver process
- Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve
- Help Individuals access services and supports they need to meet their goals
- Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed
Who You’ll Work With
Case managers work with Individuals on two Medicaid Waivers: Family Supports Waiver (FSW) and Community Integrated Habilitation Waiver (CIH).
Requirements
- Valid driver’s license, car insurance, and reliable transportation
- Bachelor’s Degree in Human Services and a minimum of one year of professional experience working with Individuals with intellectual and developmental disabilities
- Designated home office setting with computer, internet service, and smartphone
