Case Manager/Family Support Worker, Northampton
About the role
The Case Manager/Family Support Worker will provide community-based direct care services to children, adults, and families. Services include crisis stabilization, ongoing support, and case management through the state's Children's Behavioral Health Initiative. The primary goal is to alleviate mental health issues and strengthen family structures.
Responsibilities
- Provide skills training and assistance in daily living activities, including basic living, interactive, and parent skills.
- Aid in developing communication and conflict resolution skills and serve as a role model for interpersonal relationships.
- Support parents in meeting children's daily needs; observe and document child growth and development.
- Facilitate family advocacy needs; mediate as needed.
- Counsel and provide intensive support during psychiatric crises or periods of transition.
- Assist consumers in identifying and accessing appropriate mental health and medical services.
- Develop and maintain a natural support network of family and friends.
- Provide transportation for consumers as appropriate, adhering to program guidelines.
- Maintain appropriate contacts with family members, other helpers, outpatient therapists, and others to support consumers.
Requirements
- Bachelor's degree in a mental health field.
- Experience working with children and families.
- Valid driver's license and reliable transportation.
- Bilingual candidates are strongly encouraged.
