About the role

The Case Manager/Family Support Worker will provide community-based direct care services to children, adults, and families, focusing on alleviating mental health issues and strengthening family structures.

Responsibilities

  • Provide skills training and assistance in areas of daily living activities including basic living, interactive and parent skills.
  • Aid in developing communication and conflict resolution skills and act as a role model for interpersonal relationships.
  • Help parents meet children's daily needs; observe and document child growth and development.
  • Assist with family advocacy needs; facilitate "mediation" as appropriate.
  • Provide counseling and intensive support to consumers during psychiatric crises or periods of transition.
  • Assist consumers in identifying appropriate mental health and medical services and accessing services of choice.
  • Provide assistance with developing and maintaining a "natural support network" of family and friends.
  • Maintain appropriate contacts with family members, others in the "natural helping network," outpatient therapists, and others as necessary to support consumers.
  • Provide transportation for consumers as appropriate, following program guidelines.
  • Maintain accurate and timely documentation of all services provided.

Requirements

  • Bachelor's Degree in a mental health field.
  • Experience working with children and families.
  • Valid Driver's license and reliable transportation.
  • Bilingual candidates are strongly encouraged to apply.
