Case Manager/Family Support Worker, Athol
About the role
The Case Manager/Family Support Worker will provide community-based direct care services to children, adults, and families. Services include crisis stabilization, ongoing support, and case management through the state's Children's Behavioral Health Initiative. The primary goal is to alleviate mental health issues and strengthen family structures.
Responsibilities
- Provide skills training and assistance in daily living activities
- Aid in developing communication and conflict resolution skills
- Facilitate mediation as appropriate
- Counsel and support during psychiatric crises or transitions
- Assist in accessing and coordinating mental health and medical services
- Develop and maintain a natural support network
- Provide transportation and assist with daily needs
- Maintain contact with family members and other support networks
Requirements
- Bachelor's degree in a mental health field
- Experience working with children and families
- Valid driver's license and reliable transportation
- Bilingual candidates strongly encouraged
