Case Manager Crisis - CCRP

Under the assigned supervisor, the Mobile Crisis Case Manager provides community-based response, crisis intervention, case management, counseling, and mental health services to children and adults in accordance with agency policies and program guidelines. The role works to alleviate or stabilize mental health and behavioral health symptoms in referred clients that impair their ability to adjust to home, integrate into the community, or maintain living stability. This position links clients to resources that help alleviate crisis and is full time.

Responsibilities

  • Provide daytime crisis intervention services, with hours starting at 8 am–5 pm (crisis calls may extend past 5 pm until coverage from the evening shift arrives).
  • Cover nights, weekends, and holidays if schedule or staffing requires additional support.
  • Conduct Crisis/Safety Assessments with an in-person response ETA of 90 minutes.
  • Link clients to necessary resources such as health insurance, therapy, housing, and medical treatment.
  • Provide in-home services to identify service needs, assess child and family safety, develop safety plans, and deliver services to meet family and child needs.
  • Implement services under the ARC (Attachment, Self-Regulation, and Competency) practice framework.
  • Provide follow-up services after crisis response, up to or beyond 90 days, including completion of Mental Health Assessments, Treatment Plans, and other Case Management services.
  • Deliver mental health stabilization and specific services according to funding source (e.g., Medicaid) where applicable.
  • Complete required paperwork within established timelines and maintain Medicaid-compliant documentation.
  • Obtain and re-certify for the IM CANS assessment tool yearly.
  • Handle aspects of intake and assessment as assigned per program need.
  • Provide transportation for clients within program guidelines.
  • Serve as a central point of communication for service components during crisis to integrate all services provided for the client.
  • Represent the agency at court hearings as necessary.
  • Maintain comprehensive client files.
  • Perform other related duties as assigned.

Requirements

  • BS/BA degree (BSW preferred) in a human service-related field.
  • 3 or more years of related experience in child welfare.
  • Valid driver’s license and acceptable driving record with required levels of auto insurance coverage.

Skills

  • Proficient computer skills in a Microsoft Suite environment; ability to adapt and expand software application skills as needed.
  • Highly organized, flexible, and capable of handling multiple priorities/projects with tight deadlines.
  • Ability to function well in a team environment.
  • Strong project management, analytical, and presentation skills.
  • Resourceful in researching issues and developing solutions with minimal supervision.
  • Excellent interpersonal and effective communication skills (verbal, written, active listening).
  • Demonstrated ability to build professional relationships and use problem-solving techniques.
  • Ability to exercise discretion and ensure confidentiality.
