Case Manager – Concord Counseling Services: Westerville, OH
About the role

Case Managers support clients with mental illness with daily living activities to increase their level of functioning, reduce hospitalizations, and ensure their access to community resources.
Case Managers provide ongoing linkage, monitoring, planning, evaluation, and advocacy for clients within the medical and social service community, law enforcement, and health department. Responsibilities include safely transporting clients to related appointments, teaching and supporting daily living activities such as shopping, nutritious meal preparation, and budgeting of financial resources. Case Managers also monitor symptoms, de-escalate behaviors, and respond to crises involving medical and community professionals as needed.
Requirements
- Associate’s Degree in social work or a related field with a minimum of one year of direct contact care in mental health preferred.
- Working knowledge of mental illness, psychiatric symptoms, and their impact on functioning and behavior.
- Ability to meet direct client contact goals, including providing required documentation such as service notes, needs assessments, and individual service plans in the Electronic Medical Records system.
- Valid driver's license, a reliable vehicle, and insurance with a good driving record. Ability to safely transport clients.
- Ability to pass a pre-employment background check and drug screen.
