Case Manager, Community Respite

As a Case Manager in our Respite Program, you'll play a vital role in helping caregivers and families access the resources, services, and support they need to maintain healthy, balanced lives.
About the role
As a Case Manager for our Respite Program, you'll help connect children, caregivers, and respite providers with the services and support they need to thrive. This is more than a case management job—it's an opportunity to reduce caregiver stress, strengthen families, and create meaningful moments of relief and connection. Respite services provide temporary relief for youth and primary caregivers of youth with disabilities, chronic illnesses, or other special needs.
Responsibilities
- Conducting assessments and determining eligibility for respite services
- Developing and monitoring individualized service plans
- Coordinating services and connecting families with community resources
- Supporting caregivers by identifying needs and advocating for appropriate services
- Building supportive relationships with caregivers, children, and respite providers
- Maintaining accurate documentation and case records
- Monitoring program compliance and service quality
- Collaborating with community partners, providers, and interdisciplinary teams
- Providing ongoing support to help families achieve their goals and improve quality of life
Qualifications
- Associate's degree required; Bachelor's degree in Social Work, Human Services, Psychology, Sociology, or a related field preferred
- Experience working with children, families, or individuals with support needs is highly valued
- Strong communication, relationship-building, organization, and problem-solving skills
- Ability to manage multiple priorities with empathy and professionalism
- Compassionate and committed to serving others
- Comfortable working independently while collaborating with a team
- Knowledgeable about community resources and person-centered services
- Valid driver's license and ability to travel locally as needed
- Experience in case management, human services, healthcare, disability services, or a related setting preferred
