About the Role

The Case Manager & Respite Provider, under the administrative/clinical supervision of the Clinical Supervisor of Foster Care/System of Care, is responsible for providing high-quality care coordination and direct behavioral health respite services to clients and families identified by the Cuyahoga Tapestry System of Care (CTSOC) or enrolled in the OhioRISE program. This dual-role position will have a split caseload coordinating formal and informal services using the High-Fidelity Wraparound model while also delivering direct, short-term respite care to support youth in a structured, safe environment across home and community settings. Work hours include afternoon hours with required evening and/or weekend availability to accommodate respite needs and family schedules.

Responsibilities

  • Provide Wraparound Care Coordination services as part of the CTSOC, utilizing the High-Fidelity Wraparound model to clients and families identified for the project across home and community settings.
  • Complete all required assessments and documents as outlined by the agency while maintaining accurate, timely clinical records in accordance with agency quality standards, CTSOC standards, and applicable licensing/accreditation regulations.
  • Work collaboratively with identified formal and informal partners on behalf of the Child and Family Team. Act as a liaison when new services or resources need to be coordinated or developed.
  • Provide Therapeutic Behavior Services (TBS), and Community Psychiatric Support Treatment (CPST) services where appropriate on assigned cases and participate in crisis management as necessary, to meet agency productivity standards.
  • Provide short-term, direct behavioral health respite care and supervision to youth (ages 8–17) in a safe home or in the community to offer caregivers planned relief.
  • Provide transportation for youth enrolled in respite services to community activities.
  • Engage youth through companionship, structured recreational, educational, and therapeutic activities suited to their individual age, abilities, and treatment or care plan goals.
  • Maintain open, effective communication with youth, caregivers, and treatment team members, providing timely updates regarding activities, progress, and behavioral observations.
  • Respect client privacy and strictly adhere to confidentiality guidelines and HIPAA standards regarding all client-related documentation.
  • Attend scheduled staff meetings, clinical supervision, and ongoing training as required.
  • Maintain all required trainings, certifications, and licenses per regulatory guidelines.
  • Maintain high standards of ethical and professional conduct and adhere to all agency policies and procedures.
  • Other duties as assigned by management.

Requirements

  • Minimum High School Diploma required. Bachelor’s Degree in Social Work, Counseling or related field preferred.
  • QMHS certification required. Valid Ohio LSW license preferred.
  • Demonstrated expertise in systemic family therapy, crisis intervention, and family education.
  • Prior experience in systemic family therapy, crisis intervention, family education, linking/advocacy skills; across diverse environments that include: educational services, mental health field, childcare, and other community support services.
  • Must have and maintain a valid driver’s license and driving record that meets the underwriting criteria of the Agency’s insurance company.
  • Must be approved by Applewood to transport clients in own vehicle.

Pay

Salary Range: $18-$21.00 per hour depending on licensure and experience in the field.

Schedule

Work hours include afternoon hours with required evening and/or weekend availability to accommodate respite needs and family schedules.
