About the role
The Case Manager directs and facilitates the delivery of appropriate support services for participant households as indicated by relevant service modalities. The Colorado Rapid Re-Housing for Re-Entry program serves unhoused justice involved individuals and utilizes a housing-first model to assist participants in obtaining stable permanent housing.
Responsibilities
- Conduct needs-based and program eligibility assessments
- Engage in community outreach
- Provide case management services
- Program-wide service coordination
- Make demonstrable progress towards obtaining and applying knowledge and experience in field-relevant best practices and standards
Requirements
- Bachelor’s degree in human services, social work, or a closely related field or related experience
- One year of direct experience in the provision of human services
- SOAR (SSI/SSDI Outreach, Access, and Recovery) Certification
- Colorado driver’s license and state-mandated automobile insurance
- Personal vehicle for work-related travel (reimbursement for mileage is available)

Pay
$23-25 Starting Pay Based on experience
Schedule
Full-Time. Work hours vary depending upon program requirements but are typically scheduled around a 40-hour work week M-F.
