Case Manager: Colorado Rapid Rehousing for Re-Entry (COR3)

About the role

The Case Manager directs and facilitates the delivery of appropriate support services for participant households as indicated by relevant service modalities. The Colorado Rapid Re-Housing for Re-Entry program serves unhoused justice involved individuals and utilizes a housing-first model to assist participants in obtaining stable permanent housing.

Responsibilities

  • Conduct needs-based and program eligibility assessments
  • Engage in community outreach
  • Provide case management services
  • Program-wide service coordination
  • Make demonstrable progress towards obtaining and applying knowledge and experience in field-relevant best practices and standards

Requirements

  • Bachelor’s degree in human services, social work, or a closely related field or related experience
  • One year of direct experience in the provision of human services
  • SOAR (SSI/SSDI Outreach, Access, and Recovery) Certification
  • Colorado driver’s license and state-mandated automobile insurance
  • Personal vehicle for work-related travel (reimbursement for mileage is available)

Pay

$23-25 Starting Pay Based on experience

Schedule

Full-Time. Work hours vary depending upon program requirements but are typically scheduled around a 40-hour work week M-F.
