Position Details
- Hours: 37.5 hour work week; Monday-Friday 8am-4pm; with a rotational on-call schedule.
- Salary: $20/hr.
- Conduct client intakes, assessments, and individualized goal planning.
- Provide weekly case management sessions and ongoing advocacy, including referrals and support at external appointments.
- Assist clients in securing housing, understanding leases, and meeting tenant responsibilities.
- Deliver crisis intervention and support to promote client safety and housing stability.
- Maintain accurate client records, data entry, and required monthly/quarterly reporting.
- Collaborate with property management to ensure compliance with housing and program requirements.
- Build relationships with community partners and local landlords to expand housing access.
- Conduct exit interviews and complete required documentation when clients transition from the program.
- Participate in meetings, trainings, and the program’s on-call rotation.
- Perform additional duties as assigned.
- Bachelor's degree in human services or related field with two years' experience or associate degree equivalent with four years' experience.
- In-depth knowledge and understanding of the social welfare system.
- Valid driver's license, clean driving history, and reliable personal vehicle. Proof of insurability may also be required.
- Current FBI, criminal, and child abuse clearance or the ability and willingness to obtain.
