About the Role
The Case Manager provides relational support to low-income clients working towards accomplishing their goals and family sufficiency. This role offers encouragement and resources to clients and is responsible for tracking and documenting case status.
Responsibilities
- Facilitate case management of participants within program guidelines using assessment tools; develop rapport and partnership with participants.
- Develop case plans and support participants in establishing goals; meet regularly to track progress.
- Coordinate referrals and work to support desired program outcomes.
- Maintain working relationships with community agencies to stay aware of available services; promote The Salvation Army’s services.
- Coordinate services and communicate about cases; ensure valid releases of information are on file.
- Create case plans and help participants achieve identified goals through agency resources and referrals to outside resources.
- Enhance personal knowledge and skill through community and agency educational forums.
- Use an electronic database to enter, retrieve, and maintain accurate case records per protocols; manage service transactions, case plans, and assessments.
- Monitor assistance fund balances and request resources as appropriate.
- Attend all job-related training.
- Fulfill ad hoc and periodic survey requests; provide information needed for reports to administration.
- Attend staff meetings and community meetings as assigned.
- Provide emergency utility assistance for clients seeking help with DTE, including prescreening and data entry.
- Adhere to state-regulated MEAP guidelines.
- Assist 10-15 DTE qualifying clients when funding is available; help clients seeking food, clothing, and housing assistance.
- Work with Gleaners & Forgotten Harvest to oversee client emergency food programs.
- Provide seasonal assistance for clients, participate in distribution, and enter client data in TSAMM.
- Perform other duties as assigned.
Requirements
- Bachelor's degree in social work, psychology, sociology, or related field; or equivalent combination of training and experience.
- 2 or more years of applicant interview or case management experience.
- Experience working with program target population (preferred).
- Learn and adhere to The Salvation Army Social Services Code of Ethics.
- Adhere to professional codes of ethics as they apply to professional licensing.
- Understand and adhere to professional boundaries within and outside the organization.
- Maintain client confidentiality and obtain appropriate releases of information.
- Demonstrate a continued effort to increase self-awareness and better understand other cultures and issues of social justice.
- Complete Caseworker Certification Program within 120 days of hire.
- Proficient in Microsoft Office 365; basic knowledge of Teams and SharePoint.
- Proficient in SIMS (ServicePoint) software within 90 days; willingness to learn new software as needed.
- Must have and maintain a current valid driver’s license and pass the Salvation Army Motor Vehicle Record check (MVR).
- Complete Safe From Harm training and keep current.
