Case Manager

Provides case management, staffing, and life-skills programming to residents of Plover Place, a 24-unit permanent supportive housing program for single adults experiencing homelessness or at risk of homelessness.

Responsibilities

• Provide long-term strength-based, person-centered holistic case management (60%)
• Coordinate services for participants with collaborative agencies (15%)
• Assist with program fiscal responsibility including budgeting, grant spending, and billings (15%)
• Maintain up-to-date, accurate participant information in case files and through database entry (5%)
• Engage in internal and external activities/meetings to promote and further The Salvation Army's goals and objectives (5%)
• Drive agency vehicles locally to facilitate case management services
• Support The Salvation Army’s religious mission as an employee of the organization
• Perform other duties as assigned by supervisor

Requirements

• Bachelor’s degree in a relevant field (or equivalent education and experience); if degree is in social work, must be licensed by the state board
• At least one year of experience with the target population served
• Experience working in a social service setting conducting long-term case management preferred
• Must pass background checks, possess a valid driver’s license, and meet MVR requirements
• Proficient with PC, Microsoft Office, databases, and standard office equipment
• Excellent organizational skills and attention to detail
• Flexible interpersonal and communication skills; ability to communicate clearly in English
• High level of honor in dealing with confidential information; works with integrity, valuing honesty and diligence
• Able to occasionally lift 20 pounds

Working Conditions

• Variety of work including sitting, standing, walking, reaching, bending, typing, and lifting

Pay

$20.81 to $23.29 per hour, 40 hours a week
