Case Manager

Responsibilities

• Apply case management strategies to assess, plan, implement, coordinate, monitor and evaluate options and services to meet an individual's healthcare needs.
• Communicate with available resources to promote quality, cost-effective outcomes.
• Collaborate with physicians, social workers, and ancillary staff to develop and implement individualized care plans.
• Maintain detailed records and facilitate communication between all parties.
• Monitor client progress and provide support and crisis intervention when necessary.
• Oversee the discharge planning process, including in-patient length of stay (LOS), denied days, and resources utilization.
• Perform daily utilization reviews in collaboration with the physician.

Requirements

• Must possess a degree in Nursing.
• Valid RN License.
• 3 years of clinical experience; Medical/Surgical experience preferred.
• Managed care experience preferred.
• Case management experience preferred.
• Utilization review skills using MCG preferred.

Min USD $36.92/Hr. Max USD $66.46/Hr.
