Essential Duties

  • Provide individual and small group instruction in self-care, activities of daily living, communication, social and motor skills.
  • Provide direct care for ADLs and group activities.
  • Lift and transfer individuals as appropriate to their needs.
  • Cookordinate services with Interdisciplinary Team.
  • Communicate respectfully and pro-actively with families, residential staff and all members of participants’ teams.
  • Develop and implement Day Habilitation Service Plans (DHSP), including:
    • Maintain progress notes and document progress on objectives.
    • Maintain assigned participant records so that all information is current and filed appropriately.
    • Write bi-annual and annual summaries.
  • Complete all required documentation in a timely and accurate manner.
  • Participate in scheduled supervision sessions with Assistant Director.
  • Participate in program-wide staff meetings and case review meetings as scheduled.
  • Participate in team meetings regarding general program development, including curriculum development and Schedules.
  • Ability to demonstrate good computer skills as well ability to learn to utilize the agency’s computer programs for documentation of necessary data.
  • Attend in-service trainings as required.
  • Ability to work in a team environment.
  • Ability to multitask and prioritize tasks.
  • Strong written and verbal communication skills along with excellent interpersonal skills.

Requirements

  • Experience working with Individuals with ID/DD
  • A Bachelor's degree or an associate degree in a related field with two years of experience, or HS diploma and five years of related work experience in Human Services.
  • Must be able to pass Fingerprint background check, CORI, DPPC and Driving background checks
  • Valid Driver's License required
  • Valid MAP certification is preferred
  • Excellent verbal and written communication skills
