Case Management Supervisor

About the role

The Case Management Supervisor oversees daily operations in a Long-Term Acute Care Hospital, ensuring timely admission review, appropriate care levels, utilization management, and safe discharge planning.

Responsibilities

• Supervising case managers, social workers, and utilization staff
• Managing caseloads
• Conducting training
• Serving as a clinical resource
• Overseeing pre-admission screening, payer reviews, and managing denials and appeals
• Ensuring compliance with CMS and other standards
• Coordinating patient transitions for discharge planning, addressing barriers, and educating patients and families on care options
• Monitoring quality metrics, auditing documentation, and preparing reports for leadership
• Supporting continuous improvement
• Collaborating with hospitals, payers, and community providers, contributing to referral evaluations and interdisciplinary rounds

Requirements

• Nursing degree with licensure
• 2-3 years of case management experience
• Supervisory skills
• Knowledge of payer processes and clinical conditions
