Case Management Rep, Bilingual
About the role
The Case Management Representative supports delivery of comprehensive care management services within a multidisciplinary team to promote the success of the Population Health Management care model. This model aims to improve quality, reduce cost, promote optimal patient health and well-being across the continuum of care, and enhance patient and provider satisfaction.
You will implement care plan interventions as delegated by licensed case managers and Clinical Services team leaders to facilitate high-quality, cost-effective, patient-centered, and holistic care. This role involves conducting intake interviews, collecting patient-level information, and assisting with administrative support tasks. You will promote optimal health and self-management through advocacy, empowerment, multi-modal communication, coaching, and connecting patients with resources. The role may include home, community, inpatient, and/or ambulatory-based patient encounters, and you will be accountable for consistently achieving quality metrics.
Mental Health First Aid training is preferred.
Responsibilities
- Implement care plan interventions as delegated by licensed case managers and Clinical Services team leaders.

- Facilitate efficient service delivery and promote a positive patient-primary care provider (PCP) relationship.
- Engage in home, community, inpatient, and/or ambulatory-based patient encounters.
- Consistently achieve quality metrics.
Requirements
- High School Diploma or Equivalent (Required).
- Associate's Degree (Preferred).
- 2 years of experience in one of the following:
- Clinical - Acute Care (Required), or
- Healthcare - Acute Care (Required), or
- Healthcare (Required).
Qualifications
- Certified Health Coach (Preferred).
- Mental Health First Aid training (Preferred).
