Case Management Manager
About the role
Team Leadership: Manage assigned case managers, professional associates, and clerical staff to ensure productive workflows. Operational Oversight: Coordinate disease management programs and track patient outcomes throughout the post-acute period. Resource Management: Contract with internal and external healthcare resources to secure seamless transitions of care. Interdisciplinary Collaboration: Communicate regularly with administration and medical staff regarding admission appropriateness, length of stay (LOS), and discharge barriers.
Responsibilities
- Licensure / Certification / Registration: One Or More Of The Following Required Registered Nurse credentialed from the Tennessee Board of Nursing obtained prior to hire date or job transfer date. Social Worker credentialed from the Tennessee Board of Social Workers obtained prior to hire date or job transfer date. One Or More Of The Following Preferred Case Manager credentialed from the American Case Management Association (ACMA). Case Manager credentialed from the Commission for Case Manager Certification (CCMC).
- Education: High school diploma/GED with 2 years of experience, or Associate's degree, or Technical degree required. Master's degree preferred.
- Work Experience: 3 years of experience required. 1 year of leadership or management experience required.
Requirements
