Case Management Analyst - Field Richmond, VA

Responsibilities

• Facilitate appropriate healthcare outcomes for waiver/LTSS members by providing care coordination, support and education for members through the use of care management tools and resources.
• Manage a full caseload of waiver members.
• Conduct in‑person quarterly visits with members.
• Travel up to 75% of the time to meet members face‑to‑face.
• Ensure compliance with contractual requirements.

Requirements

• Minimum 2 years’ experience in behavioral health, social services, or a related field equivalent to the program focus.
• Must reside in Richmond, Virginia or surrounding areas.
• Must possess reliable transportation and be willing and able to travel up to 70% of the time (mileage reimbursed per company expense reimbursement policy).
• Experience with Microsoft Office Suite.

Qualifications

• Case management and discharge planning experience preferred.
• Managed care experience preferred.
• LTSS experience preferred.
• Bachelor’s degree or non‑licensed master‑level clinician in behavioral health or human services (psychology, social work, marriage and family therapy, counseling) preferred.
• Certification in LMHP, RN/LPN, QMHP, LMSW, LBSW, MSW, or BSW preferred.

Skills

• Proficiency with Microsoft Office Suite.

Pay

The typical pay range for this role is $21.10 – $40.90 per hour (base hourly rate or base annual full‑time salary for the job grade). The actual base salary offer will depend on factors such as experience, education, geography, and other relevant considerations. This position is eligible for a CVS Health bonus, commission, or short‑term incentive program in addition to the base pay range.
