Care Transition Liaison- LPN, RN, PT, OT,

Responsibilities

• Affords patients with guidance through post-acute care processes.
• Utilizes evidence-based clinical guidelines, preventative measures, protocols, and metrics to develop patient-centered transition plans.
• Engages in strategic partnerships with health systems, hospitals, inpatient facilities, physicians, and physician groups.
• Represents the branch in executive-level opportunities.

Qualifications

• Graduate from an approved school of nursing, therapy, or social work.
• Licensed in the practicing state.
• Two years of demonstrated field experience.
• Experience and understanding of federal, state, and local laws and regulatory guidelines governing Medicare certified home health and hospice.
• Demonstrated basic technology skills, including operation of a mobile device.

Requirements

• Valid state driver license.
• Automobile liability insurance as required by law.
• Dependable transportation in good working condition.
• Safety in operating an automobile in all types of weather conditions.
