Responsibilities

Assists patients in navigating post-acute care processes. Develops transition plans that are patient-centered and promote quality and efficiency. Represents the branch in strategic relationships with various healthcare entities.

Qualifications

• Must be a graduate of an approved school of nursing, therapy, or social work.
• Must be licensed in the state where practicing.
• Must have two years of demonstrated field experience.
• Must have experience and understanding of relevant federal, state, and local laws and regulations.
• Must demonstrate basic technology skills, including operation of a mobile device.

Requirements

• Valid state driver license.
• Automobile liability insurance as required by law.
• Dependable transportation in good working condition.
• Safety record for driving an automobile in all types of weather conditions.

Compensation

$50,000 to $75,000 annually
