CARE NAVIGATOR - Pine St
Brief Description
Works as part of an interdisciplinary team to assist patients address personal and environmental difficulties which could negatively affect treatment adherence. Care Navigators will ensure the execution of service plans by accompanying patients to medical appointments, promoting healthy living, providing social support, health education literature, conducting home visits, and community outreach, as well as ensuring engagement and obtaining health home consents.
Responsibilities
- Ensure the execution of service plans by accompanying patients to medical appointments
- Promote healthy living
- Provide social support
- Conduct home visits
- Carry out pre-visit planning
- Provide case management to address patient’s individual needs as they pertain to treatment adherence, including but not limited to transportation to medical appointments and childcare
Requirements
- Education: HS Diploma or GED required
- Experience and/or Skills: Knowledge of programs and services, ability to write professional reports and correspondence, knowledge of health care, excellent interpersonal skills, excellent time management and organizational skills, excellent computer skills required, bi-lingual, Spanish preferred
