About the role
Optum is a global organization that delivers care, aided by technology to help millions of people live healthier lives. The Telehealth FMC program is a non-clinical, non-licensed healthcare resource that conducts telephonic outreach to members who have recently discharged from the hospital. The agent provides resource assistance to members, offering information regarding transportation, medication, and primary care follow-up services to ensure timely follow-up with primary care providers (PCPs), transportation to appointments, and medication adherence.
Schedule
- Full-time, Monday - Friday
- Required flexibility to work any 8-hour shift during normal business hours of 7:30 AM - 5:00 PM PST
- Occasional overtime may be necessary based on business needs
- 1 week of paid training from 8:30 AM to 5:00 PM CST, Monday - Friday, followed by on-the-job training during the normal work schedule
Responsibilities
- Guide members on utilization of plan benefits such as follow-up with PCP, transportation, and medication follow-up
- Complete the FMC assessment in Pathway and accurately document member conversations
- Transfer members to member services when needed for follow-up
- Provide coverage for colleagues when on PTO as needed
- Other duties as assigned
Requirements
- High School Diploma / GED or equivalent work experience
- Must be 18 years of age or older
- 2+ years of experience working in a physician's office, Skilled Nursing Facility (SNF), Assisted Living Facility (ALF), or outpatient clinic with medical team communication and collaboration
- 2+ years of experience handling inbound and/or outbound calls with patients/members
- Ability to work an 8-hour schedule during the work hours of 7:30 AM - 5:00 PM Pacific, Monday-Friday
Preferred Qualifications
- Experience with Microsoft Word (edit, create, and save documents), Microsoft Excel (adding, sorting, and filtering data), and Microsoft Office (email, folders, attachments, and calendar)
- Administrative and organizational skills
- Ability to self-direct work and multitask multiple deliverables
Telecommuting Requirements
- Ability to keep all company-sensitive documents secure (if applicable)
- Required to have a dedicated work area established that is separated from other living areas and provides information privacy
- Must live in a location that can receive a UnitedHealth Group-approved high-speed internet connection or leverage an existing high-speed internet service

The salary for this role will range from $20 - $36 hourly based on full-time employment.
