About the role

This position is responsible for all functions of case management including advocacy, coordination, assessment, linking, and monitoring of services for an assigned caseload of individuals with a primary diagnosis of developmental disability or mental illness. Settings include the CMH office, home, and/or the community.

Responsibilities

  • Gathers and reviews information such as medical, psychological, and social history contributing to the individual’s situation. Evaluates this information to determine the individual’s needs and capabilities towards the least restrictive environment and/or services.
  • Initiates and coordinates professional assessments according to the presenting problem and relevant standards. Shares and presents assessment/treatment data as authorized and required for implementation and coordination with other agencies, courts, and caretakers.
  • Completes the person-centered plan of service and periodic review of progress. Ensures that the preferences of the individual served are reflected in the individual plan of treatment.
  • Makes direct/indirect contacts in accordance with the person-centered plan with the assigned individuals. Develops short- and long-term goals related to the needs presented and assures the implementation, coordination, and monitoring of the person-centered plan of service in residential, employment, school, and other settings.
  • Serves as the primary advocate for assigned individuals and as the primary liaison with their families, guardians, caretaker, and/or their agencies involved in their service network.
  • Provides consultation, referral, and in-services to staff, caretakers, families, and other agencies as needed to meet the specialized needs of individuals served.
  • Participates in and assists with placement planning and completes necessary forms and paperwork for placement. For individuals requiring specialized, contracted, and other placement services, completes and coordinates a residential treatment plan and visits the home as required to monitor progress in the placement setting.
  • Provides community outreach to individuals who have a history of hospital recidivism in order to be proactive in preventing crisis situations.
  • Participates in after-hours on-call, crisis intervention during the workday, and other duties as assigned.
  • Participates in evidence-based practices as assigned (DBT/IDDT/FPE/IPS etc.).
  • Provides care management services to veterans and their families, as assigned.
  • Completes appropriate OBRA Assessments and/or provides OBRA coordination as directed by supervisor.
  • Assists with carrying out Grant requirements as needed.
  • Provides linking and coordinating to ensure Integrated Health Care needs of individuals are addressed.
  • CCBHC Care Managers will be responsible for the coordination and linkage to external services.
  • Performs initial intakes as assigned by Supervisor.
  • Performs other duties as assigned.

Children’s Care Manager

  • Linking and coordinating of services (e.g., school, probation, DHHS).
  • Family psychoeducation.
  • Participates in the BTC committee, as needed for their individuals.
  • Works with populations 0-21 years of age with SED or IDD diagnosis.

Adult Care Manager

  • Linking and coordinating of services (e.g., SS, probation, DHHS, MRS, jail services).
  • Family psychoeducation.
  • Participates in the BTC committee, as needed for their individuals.

Behavioralist Care Manager

  • Linking and coordinating services.
  • Provides behavioral intervention support to Residential Home system.
  • Completes behavioral assessment, FBA, and behavioral plan in accordance with BTC regulations.
  • Sets up data collection methods for providers to use to monitor progress.
  • Provides summaries or other means of communication to providers, treatment team members, and BTC on progress as it relates to data measures that are reported.

Requirements

  • Bachelor’s Degree in Human Services Field from an accredited school.
  • Preferred but not required licensure as a RN, LLBSW, LBSW, LLMSW, LMSW, TLLP, LLP, LLPC, or LPC.
  • Must qualify as QMHP, QIDP, or CMHP where appropriate for population being served.
  • SST licensure for staff that meet the Medicaid requirements.
  • Staff working in the capacity of a Care Manager-Behavioralist require a BCaBA.
  • For anyone working with children, must receive 40 hours of Children’s training per calendar year and become a CAFAS rater.
  • A minimum of three years of experience with examination, evaluation, and treatment of SED Children and families is required for Children’s Care Manager roles.
  • Substance license preferred; staff are encouraged to apply for a MCBAP development plan.
  • Candidate/Employee must personally be a Veteran (discharged or released under conditions other than dishonorable) when working with Veterans.
  • Valid Chauffeur’s License if transporting individuals that receive services.

Skills

  • Language Skills
    • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.

Pay

Hourly range: $24.41–$31.43. Annual range: $50,768.19–$65,380.60.

Physical Demands

  • Regularly required to sit and talk or hear.
  • Occasionally required to stand; travel between offices and throughout building, use hands to finger, handle, or feel objects, tools, or controls; and reach with hands and arms.
  • Must occasionally lift and/or move up to 25 pounds.
  • Specific vision abilities required include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.

Work Environment

  • The noise level in the work environment is usually moderate.
  • Employee may be required to travel to outside meetings or homes as determined by caseload.
  • While performing some duties, may be required to participate in after-hours on-call and crisis intervention.
