Care Manager

Overview

This role involves coordinating patient care plans to ensure quality and timely service delivery. You will assess and evaluate health care needs, focusing on positive health outcomes through coordinated and efficient care. The position requires respectful and fair treatment of patients and their families, maintaining dignity and open communication.

Responsibilities

• Coordinate the plan of care focusing on quality, timely delivery of services, and medical necessity of patient admission.
• Assess, plan, implement, monitor, and evaluate options and services to meet individual health care needs and family support systems.
• Ensure well-orchestrated, coordinated care focused on quality, efficiency, and safety.
• Maintain an open and approachable manner treating others fairly and respectfully, preserving self-confidence and dignity.
• Seek positive health outcomes by ensuring well-coordinated care.

Qualifications

• Associate degree or higher in Healthcare.
• 1+ years of experience in Healthcare.
• 1+ years of experience in ANY of the following: Case Management, EHR Coordination, or Care Management.
• Valid Registered Nurse (RN) certification/license in the state of Kansas (KS).
• Valid BSN certification/license (preferred).
