About the role
Seeking a Care Manager to provide patient-centered care management services in an inpatient setting. This role involves assessment, care planning, coordination, resource utilization review, discharge planning, documentation, regulatory compliance, and patient advocacy.
Responsibilities
- Conduct comprehensive patient assessments and develop individualized care plans
- Coordinate care across multidisciplinary teams and resources
- Manage resource utilization and review for optimal patient outcomes
- Plan and facilitate patient discharges ensuring smooth transitions
- Document interventions accurately and ensure regulatory compliance
- Advocate for patients’ needs and preferences
Qualifications
- Associate's Degree in Nursing and valid Washington RN license
- Minimum of 2 years' acute care experience in Medical-Surgical or inpatient settings
- Preferred: Bachelor's Degree in Nursing or higher, national specialty certification, and experience in care management or utilization review
- Strong assessment, communication, organizational, and collaboration skills
