What You Will Need
- Bachelor's degree required.
- Bachelor’s degree in Nursing from an NLN accredited school of nursing is preferred.
- Minimum 2 years clinical nursing experience preferred.
- Minimum two (2) years of utilization review, discharge planning, case management or disease management preferred.
- Knowledge of InterQual or MCG criteria is preferred.
- Clinical certification, such as case management certification, is beneficial.
- Basic Life Support for Healthcare providers American Heart Association (AHA)
- Work collaboratively with a range of stakeholders and team members
- Knowledge of community resources
- Experience with Microsoft Office Suite
- Strong interpersonal and oral communication skills
- Strong computer and data entry skills
- Experience with Electronic Medical Record (EMR) platform preferred
- Proven leadership skills
- Ability to work independently, setting priorities to coordinate care plan efficiently
What You Will Do
The Continuum Case Manager RN is responsible for providing comprehensive case management and utilization management services, including discharge planning, to patients across a range of clinical areas. This role will work with the social work role as needed to ensure that any care transition gaps are identified and mitigated. Daily functions may include admission review and continued stay reviews, contacting insurance companies to provide clinical information to support admissions/continued hospital stays, referring cases to the Physician Advisor for further review, collecting QA and Avoidable Days data as directed. The Continuum Case Manager-RN will function as an integral member of a collaborative and interdisciplinary team, using assessment, implementation, coordination, monitoring, and evaluation skills. The Case Manger is accountable for a designated patient caseload and plans effectively in order to meet patient needs, manage the length of stay, and promote efficient utilization of resources
