About the role

This position is Full-time and will work 12-hour, Night shifts. Case Management is a collaborative practice including patients, caregivers, nurses, social workers, physicians, payers, support staff, other practitioners and the community. The Case Management process facilitates communication and care coordination along a continuum through effective transitional care management. Recognizing the patient's right to self-determination, the significance of the social determinants of health and the complexities of care.

Responsibilities

  • Facilitate communication and care coordination among patients, caregivers, nurses, social workers, physicians, payers, support staff, other practitioners and the community.
  • Implement and manage the Case Management process to ensure effective transitional care management.
  • Recognize the patient's right to self-determination and the significance of the social determinants of health.
  • Collaborate with interdisciplinary teams to address the complex needs of patients.
  • Assess and coordinate care plans to optimize patient outcomes.
  • Manage referrals and transitions of care to ensure seamless handoffs between providers and settings.
  • Engage with patients and families to understand their needs and preferences, and advocate for their care.
  • Monitor and evaluate the effectiveness of care management interventions.
  • Participate in quality improvement initiatives and continuous learning activities.

Requirements

  • Bachelor's degree in nursing: All registered nurses within of a bachelor's degree in nursing must obtain a bachelor's degree in nursing or higher within 3 years of hire.
  • Certified Registered Nurse Case Manager (CRNC) certification preferred.
  • 2 years of experience in healthcare related field (Acute, Ambulatory, Post-Acute, etc.) or successful completion of the Transitions in Practice (TIP) program for Care Manager.
  • Experience in the same type of nursing unit in which the CM position is available.

Qualifications

  • California Registered Nurse License.
  • Master's degree in healthcare related field preferred.

Skills

  • Strong interpersonal and communication skills.
  • Ability to work effectively in a multidisciplinary team environment.
  • Knowledge of social determinants of health and their impact on patient care.
  • Effective problem-solving and decision-making abilities.
  • Excellent organizational and time management skills.
  • Proficiency in electronic health record systems.

Pay

The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
