Responsibilities
- Provide professional, comprehensive, patient-centric care management services for at-risk patients in an acute care environment.
- Assessment and planning, coordination of care, resource utilization management and/or review, discharge planning, documentation of interventions, regulatory compliance and patient advocacy.
Requirements
- Associate's Degree - Nursing degree/diploma upon hire.
- Washington Registered Nurse License upon hire.
- 2 years of acute care experience in a Medical Surgical or Inpatient setting.
Preferred Qualifications
- Bachelor's Degree - Nursing or higher within 3 years of hire.
- National Certification in area of specialty.
- 1 year of experience in care management or utilization review in any setting or successful completion of TIPS program or Case Management Orientation Program.

Pay
The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. The base pay range is $47.73 - $89.59 per hour. Additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
