About the role

Caregivers at Providence Medical Foundation are valued and indispensable members of a collaborative care management team. This model includes patients, nurses, social workers, physicians, other practitioners, caregivers, and the community. The care management process involves excellent communication, both verbal and written, and effective resource coordination.

Responsibilities

  • Collaborate with patients, healthcare professionals, and community resources to coordinate care.
  • Facilitate communication between patients, caregivers, and healthcare providers to ensure continuity of care.
  • Assess patient needs and develop care plans that address physical, emotional, and social aspects of health.
  • Manage patient referrals and coordinate care among various healthcare providers.
  • Monitor patient progress and adjust care plans as necessary.
  • Provide education and support to patients and their families regarding disease management and preventive care.
  • Participate in interdisciplinary team meetings to discuss patient cases and develop comprehensive care strategies.

Requirements

  • Graduate of an accredited school of nursing.
  • California Registered Nurse License upon hire.
  • 3 years clinical experience in an HMO, medical group, affiliated model, hospital, or medical office/clinic setting.

Preferred Qualifications

  • Bachelor’s Degree in Nursing, or Master's Degree in a related field upon hire.
  • Certification in Case Management (CCM, ACM) upon hire.
  • Specialty Certification in Oncology, Rehab, Pediatrics, Transplant, or Wound Care upon hire.
  • 3 years experience in utilization management or case management.

Pay

The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.

Schedule

This position is full-time and operates on multiple shifts available.
