Care Manager RN - Medical Management

About the role

Caregivers in the Providence Clinical Network (PCN) are valued members of a collaborative practice model that includes patients, nurses, social workers, physicians, other practitioners, caregivers, and the community. The care management process involves excellent communication, both verbal and written, and effective resource coordination.

Responsibilities

• Collaborate with patients, healthcare providers, and community resources to develop and implement care plans.
• Facilitate communication between patients, families, and healthcare teams to ensure continuity of care.
• Assess patient needs and coordinate services to improve health outcomes.
• Manage patient referrals and monitor progress towards treatment goals.
• Advocate for patients' rights and preferences in their care.
• Participate in interdisciplinary team meetings to discuss patient cases and develop care strategies.

Requirements

• Graduate of an accredited school of nursing.
• Current California Registered Nurse License.
• At least 3 years of clinical experience in an HMO, medical group, affiliated model, hospital, or medical office/clinic setting.

Preferred Qualifications

• Bachelor’s Degree in Nursing, or Master's Degree in a related field.
• Certification in Case Management (CCM, ACM).
• Specialty Certification in Oncology, Rehab, Pediatrics, Transplant, or Wound Care.
• At least 3 years of experience in utilization management or case management.

Pay

The base pay range for this position is $57.28 - $88.92 per hour. Additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.

Schedule

This position is full-time and multiple shifts are available.
