About the role
We are seeking an RN Manager - Case Management to lead care coordination efforts, focusing on patient advocacy, resource utilization, and seamless transitions of care.
Responsibilities
- Assessing patient needs
- Developing care plans
- Implementing interventions
- Monitoring progress
- Ensuring effective communication among care teams, patients, and community providers
- Understanding social determinants of health and addressing barriers such as mental health, abuse, and resource limitations to achieve optimal health outcomes
Qualifications
- Bachelor’s Degree in Nursing
- California RN license
- 5 years of healthcare experience
- 3 years in acute care case management
- 1 year in healthcare leadership
Preferred qualifications
- Master’s in Healthcare or related field
- Certifications such as CCM or ACM
