Care Manager RN - HEMATOLOGY ONCOLOGY
About the role
The Team Based Care (TBC) Care Manager RN independently uses the nursing process to care for a group of patients safely, therapeutically, and efficiently. This role conducts shared patient visits with providers as well as independent RN visits using standardized procedures/protocols and evidence-based workflows. The TBC RN collaborates with other team members to coordinate and execute the plan of care. Demonstrates initiative in the delivery of patient care in a Team Based Care Model.
Responsibilities
- Care for a group of patients safely, therapeutically, and efficiently using the nursing process.
- Conduct shared patient visits with providers and independent RN visits using standardized procedures/protocols and evidence-based workflows.
- Collaborate with other team members to coordinate and execute the plan of care.
- Demonstrate initiative in the delivery of patient care in a Team Based Care Model.
Requirements
- Graduate from an accredited school of Nursing.
- Washington Registered Nurse License.
- 2 years Professional nursing experience.
Qualifications
- Bachelor's Degree Nursing, from an accredited educational institution.
- 1 year Experience in ambulatory care nursing.
Skills
- Initiative in the delivery of patient care.
- Knowledge of standardized procedures/protocols and evidence-based workflows.
- Ability to collaborate with other team members.

Pay
The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
