Job Description
The Case Manager RN advocates for the patient and family throughout the entire episode of care. Monitors patient’s progression intervening as needed to ensure efficient patient progress. Coordinates all aspects of discharge planning and establishes a safe discharge plan. Utilizes advanced nursing skills and knowledge to comprehensively coordinate care for a defined patient population in order to facilitate patients through the continuum of care.
Qualifications
- Bachelor’s of Nursing required
- Three (3) + years case management/discharge planning experience
- CCM certification preferred
- Active RN license in NJ/PA
