About the Role

The Care Manager LCSW and MSW provide comprehensive, patient-centered care management services in an acute care environment. Their goal is to ensure continuity of care for vulnerable patients by addressing social, financial, cognitive, behavioral, or legal barriers to access.

Responsibilities

• Assessment of patient needs
• Care coordination
• Discharge planning
• Documentation
• Regulatory compliance
• Patient advocacy

Requirements

• Master's Degree in Social Work from an accredited program
• Relevant licensure (Washington Clinical Independent Social Worker License or Associate License)
• Experience in healthcare settings, including acute care

Qualifications

• Preferred certifications such as ACM/CCM
• Understanding of healthcare terminology
