About the role
The Care Manager LCSW and MSW provide patient-centered care management services in an inpatient acute care setting. They assess, plan, coordinate care, and facilitate discharge planning to ensure continuity of care for vulnerable patients by addressing social, financial, cognitive, behavioral, or legal barriers.
Responsibilities
Responsibilities include documentation, regulatory compliance, and patient advocacy.
Qualifications
- Master's Degree in Social Work from an accredited program
- Current Washington Clinical Social Worker License or Associate License
- Experience in healthcare settings, with specific experience in acute care preferred
- Additional certifications such as ACM/CCM are advantageous
