Care Manager - Mercy Pittsburg
About the role
The Care Manager, as part of the interdisciplinary team, assesses, plans, advocates, and coordinates care from admission to discharge ensuring a safe transition post hospitalization. Duties are performed in alignment with the Mercy mission, values, and service standards. The Care Management model focuses on effective transition planning and length of stay oversight to enhance patient experience, safety, and quality of care using performance metrics and best practices.
Responsibilities
- Assesses, plans, advocates, and coordinates care from admission to discharge ensuring a safe transition post hospitalization.
- Performs duties and responsibilities in accordance with the Mercy mission, values, and service standards.
- Utilizes the Care Management model to provide effective transition planning and length of stay oversight to maintain patient experience, safety, and quality of care.
- Adopts and implements best practices and performance metrics to ensure optimal patient outcomes.
Requirements
- Required Education: Bachelor's degree in Social Work required, Masters preferred.
- Experience: 1-3 years' experience in acute care hospital setting | 1-3 years' Case Management experience.
- Licensure: Must have a current Kansas license in Social Work or plan to obtain.
- Certifications: BLS (CPR) at hire date, required, or within 90 days of hire - Certification in Case Management, Preferred.
Qualifications
- Required Education: Graduate of an accredited School of Nursing.
- Experience: 2-3 years acute care hospital setting, preferred. Care Management or Utilization Management experience preferred.
- Licensure: Current License in the state of employment.
- Certifications: BLS (CPR) at hire date, required, or within 90 days of hire - Certification in Case Management, Preferred.
