Care Manager
About the role
Directly engage with clients and their family members to support home care services and community-based care management.
Conduct professional assessments to understand physical, emotional, social, and spiritual needs, develop a care plan, and manage it to ensure stability, clarity, and peace of mind.
Engage in community activities: participate on local committees, task forces, and boards; tasks may include education, presentations, and setting up meetings with professionals.
Responsibilities
- Assess clients' needs and develop comprehensive care plans
- Communicate effectively with clients, families, and other professionals
- Participate in community initiatives and events
- Manage and update care plans as needed
- Collaborate with satellite support providers
Requirements
- Minimum Bachelor’s Degree, Master’s Degree preferred in healthcare-related fields
- Experience in care management, preferably with the aging population
- Excellent written and verbal communication skills
- Valid driver’s license and proof of auto insurance
Skills
- Strong interpersonal and communication skills
- Ability to work in a team environment
- Proficiency with computers
