Care Manager

About the role

The Osborne Association is seeking a second Care Manager to join our Elder Reentry Initiative (ERI) unit. ERI aims to enhance living conditions, transitional planning, and reentry outcomes for older adult incarcerated individuals. Care Managers will assess participants' specialized needs and connect them with appropriate services upon their release from state prisons to NYS. This role is based out of our Newburgh office but may require travel to local NYS prisons (within 35 miles) for face-to-face sessions and group facilitation of educational sessions. Candidates must obtain and maintain DOCCS clearance.

Responsibilities

• Perform participant screenings, including cognitive and geriatric assessments
• Create comprehensive transitional plans
• Provide case management to older individuals who are soon-to-be released
• Offer short-term, adaptive, strengths-based case management services for older adults reentering the community from incarceration
• Cultivate partnerships between service providers within the fields of reentry and aging
• Attend assigned training
• Perform in-person collaboration as an essential function of the job

Requirements

• Bachelor’s Degree in Social Work, or in a similar field, preferred
• Two years of case management, reentry, or transitional services work experience in criminal justice, social services, or related fields for older adults
• Experience working with the formerly incarcerated population

Pay

$29.12 - $34.23 an hour

Schedule

The position is split between in-office work, work from home, and work within NYS prisons, and is subject to change with or without notice.
