The Care Manager provides care for individuals, families, aggregates of patients, and community groups from admission to post-discharge. This role functions in both structured and unstructured healthcare settings, including environments with potential variations requiring independent decision-making. The Care Manager directs the work of staff nurses, unlicensed caregivers, and other disciplines providing direct patient care.

Responsibilities

• Provide care management for patients, families, and community groups from admission through post-discharge.
• Operate in structured and unstructured healthcare settings, making independent decisions as needed.
• Direct the work of staff nurses, unlicensed caregivers, and other disciplines delivering direct patient care.

Requirements

• A minimum of 3 years of current Emergency Room experience.
• BSN (Bachelor of Science in Nursing) required.
• Current NJ RN license, BLS (Basic Life Support), ACLS (Advanced Cardiovascular Life Support), and PALS (Pediatric Advanced Life Support) certifications.
• CEN (Certified Emergency Nurse) certification required within 1 year of hire.

Pay

The rate of pay for this position will be determined based on the candidate's relevant experience, qualifications, and skills. The minimum rate does not include bonuses, incentives, differential pay, or other forms of compensation.
