Responsibilities
- Provide day-to-day leadership and supervision for assigned Population Health staff.
- Provide operational oversight of Chronic Care Management (CCM), Behavioral Health Integration (BHI), and other longitudinal care management programs.
- Lead development and optimization of workflows, documentation standards, and clinical protocols.
- Collaborate with leadership to develop and evolve integrated referral pathways between Population Health programs.
- Monitor program performance and implement workflow improvements that enhance efficiency, patient engagement, quality outcomes, and financial sustainability.
- Provide clinical consultation and guidance for complex patient situations and care coordination activities.
- Promote utilization of Population Health Services through provider engagement and education.
- Monitor quality, productivity, documentation, and operational performance for assigned programs.
Qualifications
- Required: Current New York State Registered Nurse (RN) license in good standing.
- Required: Bachelor of Science in Nursing (BSN).
- Required: Active Basic Life Support certification.
- Required: Minimum five (5) years of progressive clinical nursing experience.
- Required: One (1) to three (3) years of supervisory or leadership experience in a healthcare setting.
- Required: Experience in care coordination, ambulatory care, chronic disease management, case management, or Population Health.
- Required: Strong leadership, communication, organizational, and problem-solving skills.
- Preferred: Master's degree in Nursing, Healthcare Administration, Public Health, or related field.
- Preferred: Certification in Case Management, Care Coordination, Ambulatory Care Nursing, or Population Health.
- Preferred: Experience with Medicare care management programs and practice transformation initiatives.
- Preferred: Experience leading multidisciplinary clinical teams.
Pay
$62,354 – $83,200.00 annually
