About the role
This position involves managing and supervising a team of care managers and caregivers, ensuring comprehensive care for individuals and families. The role includes reviewing and approving care plans, conducting assessments, and providing guidance to care managers.
Responsibilities
- Manage multiple priorities while supervising a group of up to 5 or more staff
- Support families according to program policy and caseload structures
- Review Care Plans, Reassessments and other programmatic documentation
- Actively assist with monthly team meetings
- Hold biweekly 1:1 supervision meeting with care managers, offering guidance and training
- Complete record reviews, maintaining knowledge of all caseloads of team staff
- Schedule and conduct joint visits to client homes with new Care Managers as needed
- Discuss trends in caseload and care management practices/issues
- Establish time frames and set deadlines for team requirements to be completed as needed
- Facilitate case transfers and assist with client transitions
- Assume care manager/supervisory role if a caseload should need one and as back up for team
- Work collaboratively with and assist Supervisors, QA, Training and Management staff as requested
- Complete and assist with employee performance evaluations in a timely manner
- Demonstrate command of regulatory requirements through timely and accurate completion of all required paperwork and programmatic contacts
- Ensure ongoing case record and program compliance according to established policies and time frames for completion of essential case documents
- Review billing and case notes weekly for team members and others in the department, as needed
- Absorb and act as Supervisory sign-off for Child and Adolescent Needs and Strengths (CANS) assessments in UAS
Qualifications
- Master’s degree in a health, human service or related field with one year of relevant experience
- Bachelor’s degree in a health, human service or related field with two years of relevant experience
- License as a Registered Nurse with two years of relevant experience
- Résumé of relevant expertise and experience in serving children and families in child welfare, developmental disabilities, mental health, healthcare and/or other systems as well as those receiving preventive services
- Excellent writing, communication, and organizational skills
- A passion for serving children and families with special needs
- Ability to work in-person in New York City (NYC)
- Ability to travel to other Rising Ground Sites (if required)
- Ability to travel in the community and do visits
