SPECIFIC RESPONSIBILITIES
- Supervise and support Care Managers through regular individual and team supervision
- Oversee caseloads to ensure timely documentation and compliance with regulatory requirements
- Review and maintain electronic health records (EHR) for accuracy and completeness
- Provide coaching, performance feedback, and professional development support
- Audit preparation and ensuring program readiness
- Participate in field visits, case conferences, and interdisciplinary team meetings (IDT)
- Manage staff schedules and coverage; provide direct services as needed
- Collaborate with internal teams and community partners to support client outcomes
- Schedule and track required trainings (DOH, Compliance, HIPAA, Health Homes, Relias, others)
- Ensure completion of mandated annual trainings
- Facilitate staff training and provide program updates
- Monitor and communicate policy or program changes to staff and leadership
- Provide real-time coaching, field-based supervision, and case guidance
- Deliver performance evaluations, corrective action, and recognition (with HR support)
- Promote leadership development and problem-solving skills among staff
- Maintain professionalism, confidentiality, and clear boundaries
- Support a positive, team-oriented work environment
- Demonstrate strong leadership, communication, and crisis intervention skills

WORKPLACE EXPECTATIONS
- This is an office/field-based position, Nassau & Suffolk County as well as the 5 boroughs of NYC
- All office days, staff meetings and supervisory meetings are in-person and or virtual and held at one of our two locations (Brooklyn/Dix Hills)
- New York residency required
QUALIFICATIONS
- Master’s degree preferred in Social Work, Nursing, Public Health, or related field
- Bachelor’s degree required with minimum 4+ years of experience in child welfare, care management, or a related field
- Experience 2–5 years of supervisory or managerial experience
- Experience in Health Home, care coordination, or case management programs
- Strong knowledge of DOH regulations, Health Home standards, and compliance requirements

- Strong leadership, staff development, and performance management skills
- Excellent communication, organization, and problem-solving abilities
- Experience with electronic health records (e.g., Foothold) and documentation oversight
- Ability to manage multiple priorities, deadlines, and audits
- Strong crisis intervention and decision-making skills
