About the role
The Program Coordinator is responsible for the day-to-day management of the Care Management program including assuring high-quality, family-driven and youth-guided service in accordance with the policies and regulations of the agency, the Health Homes Serving Children (HHSC), and the NYS Department of Health (DOH).
Responsibilities
- Supervises 6-8 direct-reports, Care Managers, and is responsible for ensuring that Care Managers are providing services to youth and families in accordance with Together for Youth (TFY), HHSC & NYS-DOH policies.
- Participates in and initiates program development efforts.
- Collaborates with internal and external providers.
- Provides on-call administrative oversight on a rotating schedule.
- Carries a small to moderate caseload when necessary.
- Represents Together For Youth's Behavioral Health services in the community and throughout state.
- Serves on various agency and/or external committees; acts as liaison to community networks including schools, mental health programs, OCFS and Managed Care Organizations.
- Conducts file reviews, monitors charts, and maintains compliance as per TFY, HHSC and DOH standards.
- Demonstrates understanding and application of strength-based, person-centered approach to providing services.
- Demonstrates ability to supervise from a solution-focused perspective.
- Works in partnership with the Director of Care Management to ensure integrity and fidelity of the Care Management program.
- Understands program components that affect fiscal stability.
- Responsible for assigning and monitoring Care Manager training.
- Performs other duties as assigned.
Qualifications
- Minimum of Bachelor's Degree in social work or related human services discipline. Master's Degree preferred.
- Minimum of 3-5 years of experience working with children and families required.
- Minimum of 2 years of supervisory/management experience required.
- Must have a valid driver's license and clean driving record.
- Regional Travel is required approximately 20% for the position.
- Must have access to a reliable vehicle with appropriate insurance.
- Must be able to work a flexible schedule including nights and weekends as needed.
Pay
$60,000 - $64,500 annually; commensurate on experience
