About the role

The Patient Care Coordinator (PCC) supports both clerical and clinical functions to meet the needs of the nursing department. This role works closely with the Charge Nurse, pharmacists, and clinical liaisons to oversee daily scheduling tasks, including pending patients, Start of Care (SOC) appointments, routine in-office visits, and home visits. The PCC also assists with hospital discharges and ISI SOC coordination. Additional responsibilities include triaging incoming nursing department calls and troubleshooting issues promptly.
